Families with children aged birth to five years who live in Renfrew County and the surrounding area can use Let’s Grow Clinics. You do not need a family doctor to attend and services are free through the public health unit.

You can call the Renfrew County and District Health Unit to book a Let’s Grow Clinic appointment. At the clinic visit, a public health nurse will talk with you about your child’s health and development, take measurements like height and weight, make sure they are up to date with immunizations, answer your questions, and share information or referrals for additional services as needed.

Madawaska Valley Midwives
Midwifery care in Renfrew County, offering personal support through pregnancy, birth, and the first weeks with your baby.
► Who Is This For?
Midwifery care is available to Ontario residents living in the service area and with a healthy, low-risk pregnancy.
► How It Works
To get started, you fill out the intake form on the website or contact the clinic directly. If accepted, you are cared for by the same midwifery team throughout pregnancy, labour, birth, and postpartum. Appointments include prenatal check-ups, screening, lab work, and education. Midwives are on call 24 hours a day for urgent concerns and continue caring for both parent and baby until six weeks after birth.

Collaborative Care Midwifery Clinic (Petawawa)

► Who Is This For?
Pregnant people who live in Petawawa and the surrounding area can access midwifery care through this clinic. The service is funded through Ontario health care so there is no direct cost to clients.
► How It Works
To start care you call the clinic at the number on the side of the card and speak with staff to begin the intake process. Once accepted, you meet regularly with the midwifery team for check-ups and planning, get information about preparing for labour and birth, and receive support during labour and delivery. After your baby is born the midwives continue to check in with you and your baby for follow up care during the first weeks of parenthood.

Killaloe Community Resource Centre - Bumps to Babes

Pathways MotherSafe
Confidential support for pregnant/parenting women and gender diverse people who are struggling with substance use.
► Who Is This For?
Women and gender diverse people aged 16 and over who are pregnant or parenting children under six years of age can access the MotherSafe program through Pathways. There is also a related Opioid Case Management stream that can support women who are parenting children up to age 16 who are dealing with opioid use along with other substances.

You can refer yourself by calling or emailing Pathways, and referrals from other sources are also accepted with permission. A counsellor will talk with you about your situation, do an assessment, and work with you to create a treatment and support plan. You then receive ongoing support and counselling that fits your needs, and the program can help connect you with additional services for her and her family.

Champlain Screening Outreach Program
Cancer screening access program for eligible people in Eastern Ontario who do not have a family doctor. It supports screening for cervical, breast, lung, and colorectal cancer.
► Who Is This For?
Eligible individuals in Eastern Ontario without a family doctor who are covered by OHIP and meet provincial screening criteria for cervical, breast, lung, or colorectal cancer screening.

Patients can self-refer by phone or by booking online. The first appointment is a phone call, and if an in-person visit is needed, it is arranged at one of the program’s regional locations.

Diabetes Education
Diabetes education and support program for people with pre-diabetes, Type 1 diabetes, Type 2 diabetes, or gestational diabetes.

People who are newly diagnosed or living with pre-diabetes, Type 1 diabetes, Type 2 diabetes, or gestational diabetes.

Madawaska Valley Hospice Palliative Care (MVHPC) is a free support service for individuals and their families who are faced with a life limiting condition.

Individuals living with a life-limiting illness, as well as their families and caregivers. Services are available to residents of Renfrew County and nearby communities, at any stage of illness where supportive care is needed.
► How It Works
Madawaska Valley Hospice provides hospice palliative care based on each person’s needs and situation. Individuals, families, or health care providers can connect with the hospice team to discuss available supports. Care may be provided at home, in hospital, in long-term care, or through the residential hospice program. The team works with the person, their family, and their care providers to support comfort, symptom management, emotional and spiritual care, and caregiver support. Bereavement support is also available for families after a loss.

AccessMHA
AccessMHA is a free service that helps people aged 16 and older in eastern Ontario find and connect with the right mental health, substance use health, and addictions supports and care. It makes it easier to navigate the system by giving you a single place to start so you do not have to figure everything out on your own. When you reach out, you will be connected with a trained professional who will talk with you about your situation and then help connect you to services that match what you need from a network of partner organizations.

People who are 16 years old and over living in eastern Ontario can use AccessMHA to get help finding mental health supports, substance use health care, or addictions services. Family members, caregivers and health care providers can also refer someone with their permission.
► How It Works
You can go to the AccessMHA website and fill out an online form to book a time to talk with a trained specialist or you can call AccessMHA directly. A professional will contact you to talk about what you are looking for and help connect you with the right services or supports, such as counselling, peer support, group programs, psychiatry or substance treatment programs. There is no cost to use the service.

Community members without a family physician connected to the team may be able to access some services through self-referral or appointment.

Non-insured Health Benefits for First Nations and Inuit
Health benefits for eligible First Nations and Inuit, covering medications, dental, vision, mental health care, and medical travel.
► Who Is This For?
To be eligible, you must live in Canada and be either a First Nations person who is registered under the Indian Act or an Inuk recognized by an Inuit land claim organization. Children under two years old can also be covered if their parent is eligible.
► How It Works
Once you are identified as eligible and have your NIHB card or number, you access benefits through health providers who are enrolled with the program or by submitting claims for reimbursement. Some services require prior approval before you receive them, and providers can often submit claims directly to the program, so you do not have to pay up front. If you pay first, you can use the client reimbursement process to get paid back.

Regional Geriatric Program of Eastern Ontario
Specialized geriatric care helping adults 65+ manage complex health needs and stay independent with coordinated support.
► Who Is This For?
Older adults aged 65 and older living in Eastern Ontario, and their caregivers, can be referred to RGPEO services. A referral is generally made by a family doctor, nurse practitioner or health care professional. Still, in some cases, older adults or family members can ask about a referral with consent.
► How It Works
To access services, you or your health care provider can contact the Central Intake team to start a referral. A trained assessor will review your situation and connect you with the exemplary service best suited to your needs, such as an in-home assessment, outpatient clinics, day programs, or medication review support. Once you are connected, the RGPEO team works with you and your care partners to support your health goals and help you remain independent.

You do not need a referral and the services are available at no cost.

To access Registered Nurse support you can book an appointment online (which is preferred) or call the team to speak with staff. When you arrive for your appointment, the nurse will check in with you, review what you need done, and provide the appropriate care or service. These nursing supports are part of the full suite of programs offered by the Family Health Team to help you stay on top of your health.

To get nutrition support you can contact the Family Health Team office to ask about meeting with the dietitian. A staff member will help you set up an appointment so you can talk with the dietitian about your eating concerns and goals. Older Adults and/or their caregivers. Self-referrals are accepted and can be made by phone directly to the Community Geriatric Coordinator. Referrals may also be made by the hospital at discharge, primary care providers or other services providers.

The community geriatric coordinator provides social work support to older adults and caregivers with an emphasis on dementia care. Areas of service can include: community service navigation, referrals and advocacy, mood support, future planning, and adjusting to a new diagnosis.

Adults and children who live in eastern Ontario and do not have a regular family doctor or nurse practitioner can use the ERVCC. You can reach out on behalf of yourself or a family member.

You access the clinic by visiting the ERVCC website and starting a booking request online or by calling the contact number. After you register, you choose how you want to connect (phone or video) and book an appointment with a nurse or provider. At your visit you talk about what’s going on, receive assessment and care recommendations, and get guidance on follow-up, referrals or prescriptions if needed.

Diabetes Education – St. Francis Memorial Hospital
The Diabetes Education program at St. Francis Memorial Hospital in Barry’s Bay helps people living with prediabetes, Type 1 and Type 2 diabetes learn skills for lifelong diabetes care. A team including a registered nurse and a registered dietitian offers group education sessions and individual support so you can understand how diabetes works, how to monitor your blood sugar, healthy eating, medications, physical activity and how to prevent complications.
